Wellness Pointe is an FTCA Deemed Federally Qualified Health Center (FQHC) and 501(c)3 nonprofit organization. We provide a variety of medical and social support programs to benefit the health of the whole family. If you do not have a primary care physician or would like to change providers, Wellness Pointe is ready to serve your health care needs! We have nine convenient locations in Longview, Tyler, Mount Pleasant, Kilgore, and Gilmer with early morning and evening appointments available to accommodate your busy schedule.

Wellness Pointe happily accepts Medicaid, CHIP, and Medicare (including Health Spring).  We also accept most health, dental, and vision insurance including Aetna, Blue Cross/Blue Shield, TRICARE, and United Healthcare, to name a few.

- Hospice is a specialized approach to care which combines a team of medical expertise and compassion for those facing a life-limiting illness. Hospice is a benefit of Medicare, Medicaid, and some private insurance, which brings both you and your family peace, comfort, and control.

- Hospice can be provided wherever the patient lives.
- Hospice focuses on quality of life and providing comfort, care, pain management, and family support.
- Nursing home patients are also eligible for hospice care.

**¿Qué es un hospicio?**
- El hospicio es un enfoque especializado de atención que combina un equipo de experiencia médica y compasión por quienes enfrentan una enfermedad terminal. El hospicio es un beneficio de Medicare, Medicaid y algunos seguros privados, que le brinda paz, comodidad y control tanto a usted como a su familia.

**Datos importantes sobre el hospicio:**
- Se puede proporcionar cuidados paliativos dondequiera que viva el paciente.
- El hospicio se centra en la calidad de vida y en brindar comodidad, atención, manejo del dolor y apoyo familiar.
- Los pacientes de residencias de ancianos también son elegibles para recibir cuidados paliativos.

The goal of the Tyler Resource Center is to provide a one-stop-shop of wraparound services with a food pantry, benefits assistance and other support services.

Since October 1988, the East Texas Food Bank, a 501(c)(3) nonprofit organization, has provided food to over 200 partner agencies and food programs, in an effort to feed children, the working poor and seniors throughout 26 counties in East Texas. A member of Feeding America, The East Texas Food Bank is one of four food banks that serves the Gulf Coast Region who are pre-staged with food products from Feeding America, to respond to disasters.

Our food sources include a variety of national, regional and local food manufacturers, brokers, wholesalers, and retailers donate food to the East Texas Food Bank. Donated food includes overproduced or discontinued items, frozen foods, dairy products, mislabeled goods, dented cans, day-old breads and pastries, and odd-sized produce. Local farmers and gardeners provide fresh produce. Individuals, businesses and community organizations support the East Texas Food Bank through various canned food drives throughout the year.

Since Focus on the Family’s primary reason for existence is to spread the Gospel of Jesus Christ through a practical outreach to homes, we have firm beliefs about both the Christian faith and the importance of the family. This ministry is therefore based upon six guiding philosophies that are apparent at every level throughout the organization. These “pillars” are drawn from the wisdom of the Bible and the Judeo-Christian ethic, rather than from the humanistic notions of today’s theorists.
